Each person can generate about one million gigabytes of health-related data - equal to more than 300 million books - across his or her lifetime. Therefore, how to help healthcare providers to filter the most meaningful and actionable data is very critical for our health and wellness.
The primary aim of this project is to develop a Health Information Exchange (HIE) tool which can not only to help providers better understand patients’ health condition and history holistically, but also help patients to document their care journeys and navigate their care transitions in the complex healthcare system.
